Dr. Konakondla emphasizes that peridural membranes are real and intradural fat is not a normal finding. If fat is visualized between tissue layers, it indicates that the tissue is not dura. After adequate coagulation, these layers can be carefully dissected and safely removed. First State Surgery Center (FSSC) partnered with Arthrex Spine to perform the first ultra-minimally invasive (MIS) endoscopic spine surgery in the state of Delaware under Kalin Fisher, MD, First State Orthopaedics. The 27-year-old patient sustained a back injury in May 2024, while lifting something heavy. Clinical examination revealed sciatic symptoms along the S1 distribution, and further imaging diagnosed an acute disc herniation at L5-S1. After multiple rounds of physical therapy, oral medications, and injections failed to provide lasting relief, the patient underwent an endoscopic microdiscectomy at FSSC. After surgery, the patient reported immediate pain relief upon waking from anesthesia. Dr. Fisher reported that at the patient’s first post-op follow-up strength, numbness and pain were all improved. “My goal is always to perform the most minimally invasive surgery possible for patients,” said Dr. Fisher. “Endoscopic spine surgery brings that precedent to a whole new level. The technology allows us a make an incision smaller than a centimeter and get straight down to the spinal canal, essentially eliminating the need for muscle and soft tissue dissection*. There is less bone removal needed with this technology, as well.
