Hospital closures harm our local communities and reduce our access to health care. They put our own individual health and well-being at risk, particularly pregnant and parenting families. They also threaten our ability to cope with emergencies such as pandemics and natural disasters. Local communities should play a role in developing ways to ensure that people will still receive the hospital-based care we need, particularly in a sudden emergency like a heart attack, stroke, mental health crisis, childbirth or life-threatening injury. The New York State Legislature passed the Local Input in Community Healthcare Act (S.8843A/A.1633B last spring. This bill will: improve public notice and community involvement when hospitals propose to close entirely or shut units like maternity, emergency or mental health services, strengthen government oversight of proposed closures and ensure that a Health Equity Impact Assessment is done, to inform state regulators. In care, effective and open communication is paramount, especially when it comes to addressing family concerns and complaints. Several providers that adopt a "three-tier” complaint system characterised by bureaucratic layers, can unintentionally exacerbate the distress of those it aims to serve, highlighting the need for a more direct and empathetic approach. CCFTV regularly witnesses how these traditional complaint processes can break down, leading to prolonged resolution times and increased frustration among complainants. There is a need for a shift towards streamlined communication channels to enhance the way healthcare settings handle complaints. The three-tier system, designed with intentions of thoroughness, often leads to delays and increased frustration for families seeking answers or redress. Navigating through multiple levels before any substantial action is taken not only delays justice but can also dilute the original complaint. Furthermore, the impersonal nature of a tiered system can leave families feeling undervalued and unheard at times when empathy and swift action are most needed. CCFTV regularly supports families through these challenges, advocating for a complaint system that prioritises direct communication and human connection over procedural steps. These experiences highlight the need for healthcare services to adopt models facilitating direct dialogue between complainants and decision-makers, possibly through dedicated complaint resolution teams with the authority to address issues promptly and effectively. Such teams should be adept in conflict resolution and trained in empathetic communication to ensure families feel supported from the outset. Additionally, care providers need to better understand where the complaint process is most prone to breakdowns and how to prevent them. We evidence providers that implement a culture of openness and continuous feedback and see how potential issues can be identified and addressed before escalating into formal complaints. By moving away from the three-tier system and adopting a model that emphasises direct, empathetic communication, care services can promote a culture of trust, leading to quicker resolutions and a more supportive environment for families deserving of a more streamlined and transparent complaints process.#ComplaintsProcess #HealthCare #EffectiveCommunication

The primary mandate of healthcare providers and public health workers is simple: do no harm. However, healthcare systems are constantly doing harm by facilitating and enabling criminalization. While assumed to be a benevolent force, the medical system has roots in the same carceral logics of policing and punishment. From the transatlantic slave trade to the eugenics movement, medicine is central to historic systems of oppression. Today, we continue to see the medical system’s participation in criminalization through the family policing, criminal legal, and immigrant deportation systems. Everyone deserves to access health care services without fearing for their safety or the safety of their family. To truly end harm and criminalization, we must remove all forms of policing from healthcare. In 2022, Interrupting Criminalization, Movement for Family Power, and other partners co-developed the Beyond Do No Harm Principles. This resource lays out 13 actionable principles for healthcare providers and public health workers to reduce and eliminate the harms of criminalization by supporting people’s agency, self-determination, dignity of risk, and general well-being.
