We are seeking highly motivated individuals to join our local care team as a Health Coach! Diverge Health is looking for highly empathetic, personable, and humble Health Coaches to help us care for and coach our patients. Under the supervision of a Nurse Practitioner, as a Health Coach, you will visit patients in their home and conduct phone sessions to provide educational health coaching on how to manage chronic conditions. In addition, you will partner with patients to help them identify and problem solve barriers that are inhibiting their health.
Requirements
Ability to make home visits to Diverge patients
Ability to attend in-person meetings at the Diverge office twice a week (minimum)
How You Will Be Trained
Diverge prepares candidates to become Health Coaches through a full-time, paid training period, which is followed by continual learning support & dedicated clinical supervision.
12-week training program that requires 2-3 days per week in our local office
Upfront Training to become a Health Coach consists of:
Instructor-led lessons
Teach-back sessions: putting your knowledge into practice
Case Studies: to prepare you for real situations
Assessments: Oral, written, and post-unit assessments
Outbound calls to patients to begin building your patient caseload and delivering services
Job shadowing and other on the job-based learning
1:1 and group-based successes and challenges sessions to review progress and feedback
What you will be trained on:
Chronic Condition Management on Diabetes, Hypertension, Asthma, COPD and CHF
Motivational Interviewing skills in how to build rapport and influence behavior change
Conducting telephonic outreach
How to use the software to document notes, communicate social and clinical needs to your care team, and how to manage appointments/tasks/patient panel
Personal and patient safety protocols for patient home visits
What You Will Do As a Health Coach
Health Coaches at Diverge Health provide one-on-one coaching sessions to help patients implement realistic, culturally appropriate lifestyle changes. Sessions are held in the home, clinic, or convenient community settings. Our Health Coaches have a unique ability to build trust, identify barriers, and motivate individuals to achieve goals that result in reducing avoidable patient visits to the emergency room. All aspects of our care support are conducted collaboratively with the patient’s primary care provider.
Key Responsibilities
Cold call patients to engage them in our Chronic Condition Health Coaching Program
Visit patients in their homes, or community settings, to provide motivational health coaching on how to manage health conditions at home and the importance of engaging in routine preventive care visits to keep diseases in check and avoid medical risks
Communicating health complications and risks to Nurse Practitioner for review
Connect patients to local resources to help address social, emotional, behavioral and environmental barriers to health
What You Bring
2+ year of professional experience in a position that requires regularly interfacing with members of the public in an educational or customer service capacity
1+ years experience in the healthcare field preferred
Basic Technology skills and or data entry skills; preferred experience in Microsoft Office Suite, and/or Customer Relationship Management or Sales Engagement platforms
Experience with meeting quotas or KPIs
Ability to make outbound phone calls comfortably with a desire to engage in conversation;
Preferred cold call outreach experience
Ability to receive feedback and apply learnings
Associates degree preferred

Physical Requirements:
Ability to travel within the community; must have a valid driver’s license and car insurance, and access to reliable transportation for patient home visits and community outreach.
Lift and carry materials and supplies.
Stand, walk, and move for extended periods while conducting community visits or health education sessions.
Adapt to varying environmental conditions (both outside while traveling the community and inside patients' homes)
Compensation is $24/hr with a 10% annual bonus
What is Diverge? We support primary care practices in delivering and providing high-quality care with a focus on people with Medicaid insurance. We are supporting them in a transition to value-based care where doctors are paid by how healthy their patients become rather than how many patients they see. The way we support it is provide Nurse Practitioner-supervised health coaches to work closely with patients to improve their health, and flag clinical concerns, such as symptoms and medical risks.
