Use Case Document For
BootFitting Form

Acceptance criteria: As a user I should see
Bootfitting form option on right corner of the
Homepage as given in the image.

As a user when I click boot fitting form in the
homepage it should get navigate to this page.
Basic details should be changed to
customer details.
Calender should be user friendly.
Should be bigger in size
Similar like order/return module , details of
customer fields should be present in this
module.
When Birth date is selected Age should be
automatically generated.
Age should be visible. It should not be
hidden.
Dropdown list box should be provided to
select the gender.

As a user after filling up the customer
details the page should be navigated to
next page.
This is the measurement page of the
patient.
Fields should be present for both left and
right leg and the measurement of H-T(heal
to toe) and Measurment of H-B(heal to
Boe) should be entered.
Width option should be alphabetical in
character.
If any of the diseases found on any left or
right leg either yes/no option should be
selected.
If yes is selected required images should
display and the required degree should be
selected on the respective leg.

•As a user I should see diseases given below
•Hallux limitus/rigitus
•Calluses
•Raised navicular
•Pin and needles
•Calcaneal test
•Knee pain
•Onychomycosis
•Planter fascia pain
•Hallux abduction
•Hammer toe
•Diabetic
•Pregancy
•Details when any of the diseases found images
should be resized according to the measurment
which is given below and following report
should be generated.

As a user when I found patient has Hallux
Limitus/ rigidus diseases on any of the leg
Yes should be selected.
Required image should pop up and
corresponding degree should selected.
Before selecting

As a user I should scroll corresponding to
patient fingers measurment which is
present.
Final report should be generated as
(Gender - He/She) has Hallus
Limitus/Hallux Rigidus of (Gender -
his/her) (Left/Right or Both) foot and
(Gender - his/her) angle of toe lift is (%
value)

As a user when I found Calluses diseases on any of the leg yes should be selected and
images should pop up as given above. In the requied spot when calluses found it should be
selected and green color should be changed to red color.
The final report should be generated as -On (Gender - his/her) (Left/Right or Both) feet,
he has calluses(bottom view) under the Toe1, Toe 2, Toe 3, Toe 4, Toe 5, Metatarsal head,
back of the heel and medial edge of hallux. He has corns (Top view) on Toe1, Toe 2, Toe 3,
Toe 4, and Toe 5
Calluses:

As a user when I found Raised Navicular on
any of the leg Yes option should be selected
(Gender - He/She) has a prominent navicular
on (Gender - his/her) (Left/Right or Both)
feet
Raised Navicular:
Pins and needles:
As a user when I found Pins and needles on
any of the legs yes option should be
selected.
(Gender - He/She) complained of regular
Pins & Needles on (Gender - his/her)
(Left/Right or Both) feet

Calcaneal Test:
As a user when I found calcaneal diseases yes
option should be selected.
The Final report should be generated as
We carried out a calcaneal stability test and
found that (Gender - He/She) has
hypermobility of the calcaneal/Tulus joint
which is contributing to the excessive
distention of (Gender - his/her) feet. (Gender
- his/her) foot distention ratio is (X%) on his
left foot and (y %) on his right foot , this may
be a contributing factor to a scoliotic rotation
of the spine and subluxation of L4/L5.

Knee Pain:
As a user when I found knee pain diseases yes should be selected and inages should pop up
as given. Selected knee pain spot should be changed from green to red.
Final report should be generated as :
On (Gender - his/her) (Left) knee (Gender - He/She) presented with pain on the lateral
side, medial side, under the patella and back of the knee. On (Gender - his/her) (Right)
knee (Gender - He/She) presented with pain on the lateral side, medial side, under the
patella and back of the knee.

Onychomycosis:
As a user when I found onychomycosis diseases yes option should be clicked, images will pop
up and selected spot should be changed from green to red.
The final report should be generated as:
On (Gender - his/her) (Left/Right or Both) he presented with Onychomycosis of Toe1, Toe 2,
Toe 3, Toe 4, and Toe 5.

Planter Fascia Pain:
As a user when I found this diseases yes
option should be clicked.
The final report should be generated as
(Gender - He/She) has been suffering from
planta facia pain on (Gender - his/her)
(Left/Right or Both) feet especially after
extended use.

Hallux Abductus :
As a user when I found this diseases yes option should be clicked in the corresponding leg and
image should popup . Desired degree should be scrolled according to the patient leg.
Final report should be generated as:
It can be seen that (Gender - his/her) (Left/Right or Both) hallux is abducted by (%) .

Hammer Toe
As a user when I found this diseases yes
option should be clicked in any of the
corresponding legs.
The final report should be generated as
On (Gender - his/her) (Left/Right or Both)
he presented with hammer toes on Toe1,
Toe 2, Toe 3, Toe 4, and Toe 5.
HALLUX DEFORMITY SHOULD BE DELETED
DIABETIC & PREGNANCY:
As a user when i found diabetic and
pregnancy yes option should be clicked.
Height&weight:
There should be three fields for height
1.Feet 2.inch3.cm
Three field for weight:
1.Stone 2.LBS 3.KG
When cm and kg are entered it should
automatically calculate remaining fields
like feet inch for height, stone lbs for
weight.

Differential toe temp
As a user when I found this diseases I should
slect either large or small in corresponding
legs.
The final Report should be generated as:
There seems to be (Significant (Large), some
(small) )temperature difference between
the proximal and distal areas of (Gender -
his/her) (Left/Right or Both) . We
determined this to be caused by his
biomechanical misalignment which is
causing pressure occlusion on his toes.
Other condition:
As a used i should see the text box for other
conditions to be entered.
Your description:
As a user i should see the text box for
entering description of the patient disease.

After entering and selecting options in all
fields, when save option is clicked
Treatment Recommendation
Customer Decleration
options are enabled.
When treatment option is clicked the page
should navigate to next page which is shown.
The distension Ratio should be calculated
according to the formulae given below.
(will be said later)
Every column should be spaced out equally.
The laser weighted,laser
spine,jellybath,plaster of paris,thermo
molded,off the shelf,Remold,print isurance
should be aligned as grid as present in
homepage.

As a user when I click Laser weighted grid
the page should navigate to next page
and treatment cost should be displayed.
The cost should be mentioned in pounds
and pound symbol should be displayed in
every treatment options.
The total pound should be made darker in
color and should be visible.
At Top right corner edit option should be
changed to save option.
(All treatment title will be said later)
At bottom of the page user should see
Sport orthodics
Dress orthodics
Solcus orthdics
At header sport orthodics name should
have a checkbox by which customer can
have option to select sports orthodics.

Print insurance should be fixed.
When Selecting Back option the page should navigate to treatment Recommendation
page.
Not to main page of creating customer.
This condition should be displayed for all page in treatment Recommendations.

Customer declaration:
As a user I should see customer declaration
on bottom left side of the page near to
Treatment Recommendation.
After clicking Customer Declaration a page
should popup.
Title should be
Zero-Rated VAT Eligibility Declaration.
Entire page should be aligned properly.
Below Declare that Four fields should be
deleted.
Below Altimus Declaration Three fields
should be deleted.
In Declare that option above the larger text
box signature name should be displayed and
it should not be moved when customer
signs.

In Altimus declaration option Above the
larger text box signature name should
be mentioned and Required user sign
should be displayed in default.
Store performance Questionnaire text
should be deleted.
When Clicking Accept the page should
navigate to terms and conditions page.
(The terms and conditions text will be
issued later).

At Homepage When a customer name is
entered at search box and selected the page
should navigate to the page shown.
Here Special order system should be changed
to order/return history
Meeting application should be changed to
appointment history
Medical application should be changed to
medical history.
All those three field should be aligned in grid.
When clicking special order system the the
page should navaigate to another page.
But when clicked back it should navigate to
special order system grid page not to customer
info page.
Similar condition for Meeting application.

When clicked Medical Application the page
should navigate as shown.
Here Doctor name should be changed to
practitioner name.
This page should contain following Requirment.
Previous measurment
Customer Medical Report.
Customer images
Patient History( later to be discussed)
Customer Images:
As a user when I create customer data and
saving those information Treatment
Recommendation and customer Declaration
will be enabled.
Above the customer declaration Take a photo
option should be displayed. When photo is
taken those images should be displayed in this
respective customer image.
When View option is clicked it should
display is shown below.

Customer Medical Report
As a user when I found disease for respective customerthe reports are generated here in
medical application grid.
It should have a option to send Email And option to get Print.
Insurance option should be displayed here which is displayed in treatment recomendation.
The Report Format should be as given below
Altimus Details (Like letter Head, address details and logo)
Reference : (Customer Name)
Date : (Date of Assessment)

To help support the foot and help with the biomechanical problems we prescribed and manufactured a pair of (Type of Orthtoics Choosen) for his
shoes . These orthotics cost a total of (Total Cost). We believe they will reduce the likelihood of his future biomechanical problems.
