This is an excellent initiative, and long overdue. Time has taught me to plan for the worst and hope for the best with these things but, as I'm working on one of the the leading clinical data repositories and have already shown FHIR interoperability with leading EPR/EHRs, I agree with the hypothesis and look forward to seeing it tested. #CDR #clinicaldatarepository

I'm often asked what is the best way to learn FHIR and other HL7 standards? Are there any books, videos and/or courses I can take? Certainly there are. A cursory search on Google, Amazon or YouTube will take you to a plethora of emerging educators on HL7 standards and beyond. However, in my estimation, the number one way you can learn FHIR and other HL7 standards is to become a member of the HL7 community. There have been times I have learned through mentors things about FHIR I could never have learned on my own, or at least, not after many, many, many trials and errors. Although trials and error remain a key way to learn any new subject (don't worry, you'll always have aplenty) learning a new subject with the help and guidance of a seasoned veteran who imparts his/her/their wisdom is priceless. I always reiterate I was only able to get myself up to speed due to the fact that the mentors I had in the HL7 community freely gave all they learned from their years of experience along with their personal wisdom and insights. At the recent Health Level Seven International Dallas Working Group Meeting (WGM), our HL7 co-chairs and other volunteer leaders serving on HL7 councils were gifted their very own miniature HL7 HIE servers made of LEGO® parts (after all, HL7 standards are everywhere). Volunteer leaders are critical to the mission and vision of HL7 as they bring their expertise, dedication, and insights that drive the development and implementation of HL7 standards such as FHIR. These passionate individuals contribute their time and knowledge at meetings such as the Dallas WGM to ensure that HL7 standards are robust, innovative, and reflective of real-world needs. By fostering a community of professionals from various backgrounds, volunteers help HL7 achieve its goal of improving healthcare interoperability globally, enhancing patient care, and promoting efficient healthcare delivery systems. Their commitment and collective efforts are the backbone of HL7's success and continuous evolution. CMS recently made changes to the Medicare Outlier Reconciliation process. If you are a "New Hospital" as defined by CMS, you might want to pay close attention to this, as all new hospitals with cost report periods beginning on or after 10/1/2024 will be required to have an outlier reconciliation performed by their MAC. For all other IPPS & LTAC hospitals, the criteria related to the Cost to Charge Ratio (CCR) differential from the paid CCR to the Final CCR, which triggers the outlier reconciliation, will increase from 10% to 20% for the same cost report begin date as the new hospital effective date. #transmittalR12594CP #changerequest13566 #medicareoutlierreconciliation

Many healthcare organizations continue to rely on “legacy” systems powered by HL7 v2. If you’re building anything that needs to connect with these older systems, you’re likely to encounter it. Even some new systems in specific domains only offer v2 interfaces. Setting up a v2 interface can be expensive—both in time and money—but completely replacing these systems is often even more costly and disruptive. That said, for many healthcare orgs, maintaining HL7 v2 alongside newer standards is the most practical option. I KEEP SAYING IT - YOU. CAN'T. TRUST. SOFTWARE. '200,000 medical letters had gone unsent due to widespread problems with NHS computer systems. Nearly half of hospital trusts with electronic patient systems reported issues that could affect patients.' You can't trust the actual software, and you can't trust many medical staff to be bothered to check it. On the occasions when either I or my other half have had the misfortune to be taken to A&E, I have always take with me three copies of my or her medical history and medications on a sheet of A4, which I keep maintained year-by-year and/or condition-by-condition. Three copies - why? Well, I give one copy to the ambulance person. If I'm lucky, they will pass it on to the A&E department - or they will lose it. If that is the case, I give the second copy to the triaging nurse. If I'm lucky, she will pass it on to the A&E doctor - or she will lose it. That is where the third copy comes in - when the first two are lost, then at least I can give the doctor the third one. On the matter of NHS software, well read the article ........
