Our last analysis of the STRONG-HF trial describes differences in patient characteristics between early and late enrolees but shows consistent feasability, safety, and efficacy of the high-intensity care strategy of GDMT uptitration after acute heart failure. Pharmacists in the Veterans Health Administration (VHA) who received feedback on their patients' heart failure medication rates, along with educational tools, significantly increased the frequency of managing heart failure and adjusting medications for their patients. The PHARM-HF A&F Study involved 120 VHA primary care pharmacists responsible for over 7,000 heart failure patients. The pharmacists were divided into three groups: a control group receiving educational materials, an audit and feedback group, and a third group receiving targeted patient information. Preliminary findings showed that pharmacists who received audit and feedback increased their patient visits for heart failure care by 1.2 per month and medication adjustments by 0.2 per month, compared to the education-only group. They were also more likely to prescribe mineralocorticoid receptor antagonists, historically underprescribed for heart failure. 'We found that when pharmacists participated in the audit and feedback group, they were more likely to identify patients who would benefit from medication adjustment, set up new appointments with patients to adjust heart failure medications and adjust heart failure medication therapy during appointments,' said lead author Dr. Alexander Tarlochan Singh Sandhu. The study highlights the potential for practical interventions involving pharmacists to improve heart failure and chronic disease management. Disparities in Post-Hospitalization Care: The Road to Recovery is Uneven A study recently published in *Annals of Internal Medicine* highlights a crucial issue: disparities in post-hospitalization care are leaving some patients behind. While we often focus on the quality of care inside the hospital, the care after discharge is equally critical for recovery—and this is where many patients from marginalized communities face significant barriers. The study reveals that patients in underserved populations are more likely to experience poor transitions from hospital to home, resulting in higher rates of readmission and adverse outcomes. Factors such as limited access to follow-up care, cost of medications, social and family support, and understanding of discharge instructions are contributing to these disparities. Equitable healthcare doesn’t end when a patient leaves the hospital. We must prioritize improving post-discharge care and ensure patients are supported in their recovery journey.
