Meet Robert. Robert was diagnosed with heart failure during a February 2024 hospitalization. After a diagnosis like this, many patients do not have access to the tools they need to be and stay heart healthy. Through a referral to our Community Health & Well-Being's Congestive Heart Failure Program co-led by Marielena "Mari" Vega, Robert enrolled in one-on-one care delivered right to his home to support him in his health journey. "The first time I met Mari, she was an awesome person who was a great help. She helped me get transportation and getting to cardiac rehab," Robert shared. Robert was a stand-out student of the 6-month program, attending classes on nutrition and cardiac rehab, along with 90 percent of his follow up appointments, which are all critical steps to supporting long-term health. Robert is an advocate for his own health, and we're so happy to see his success!
