PFCCpartners is committed to building a community of health care providers, administrators and patients and families coming together in partnership to improve the quality, safety, experience and delivery of health care. WHAT IS PFCC? Patient & Family Centered Care recognizes that the quality, safety and delivery of health care is improved when the expertise of health care providers is partnered with the experience of patients & families. From the bedside to the boardroom, patient & family centered care is about partnering to design policies, programs and individual care plans for the best possible outcomes for patients, their families and health care providers. WHAT IS THE IMPACT OF PFCC? For patients, PFCC means that patient needs and priorities for their own health will be heard by a health care team who will respect and value them as an active participant in their own care. Empowering patients and families to be confident and comfortable in the health care environment sets the table for true collaboration. Health care organizations across the country are developing the policies, programs and facilities to build partnerships between health care providers and their patients. Within the care giving relationship, PFCC provides a basis of respect where patients & families can provide essential information, which directly impacts the clinical outcomes. In the context of health care organizations, partnerships with patients & families ensures the patient will be kept at the center of every interaction within your health care organization.

This is a unique opportunity for people with lived experience to build their skills through training and peer-learning, connect with their peers across the country, and grow their skills toward improving #healthcare for themselves and their communities. For this year’s cohort, we are looking for people who are eligible for both #Medicare and #Medicaid (“dually eligible”) and family caregivers of people who are dually eligible who are interested in building skills and confidence to advocate and use their lived experience to impact healthcare #policy.     LeadingAge is hiring! These are temporary, full-time roles through June 30, 2027. Positions are hybrid and will be based in Washington D.C. Director, Moving Forward Coalition: The Moving Forward Coalition director will lead the collaborative development and execution of a coalition-wide policy agenda. The individual will manage and direct the steering committee in identifying and building consensus on policy issues, crafting and endorsing statements, developing advocacy strategies, and executing that strategy. They will track relevant policy developments, conduct focused research, and develop public- and policymaker-facing materials. They will build partnerships and coordinate action to achieve advocacy goals. The director will work closely with the project manager on social media and communications (developing and reviewing content). They will manage the coalition’s engagement in conferences, events, and media engagement—coordinating these events and opportunities with the co-chairs and other participants. The salary range for this D.C. based position is $98,999-$104,210 based on a 37.5-hour work week for this exempt position. Salary offered may vary depending on relevant factors as determined by LeadingAge, which may include, but are not limited to, background and experience, knowledge, skills and abilities, certifications and licensures internal salary ranges, geographic location and other organizational needs. Salaries for candidates hired outside of the DMV will have salary adjusted using Economic Research Institute’s, Geographic Tool Assessor. Manager, Moving Forward Coalition: The manager is responsible for the successful management and operation of the coalition’s grants, the effective coordination of its constituent teams and initiatives, and its consistent and impactful communication with stakeholder groups. The individual will oversee and manage the Coalition’s grants, contracts, and operations. They will oversee budgeting, invoice processing, and purchasing; provide technology and administrative support across projects and initiatives; and produce the Coalition’s communications and weekly content development. The salary range for this D.C. based position is $77,332-$81,402 based on a 37.5-hour work week for this exempt position. Salary offered may vary depending on relevant factors as determined by LeadingAge, which may include, but are not limited to, background and experience, knowledge, skills and abilities, certifications and licensures internal salary ranges, geographic location and other organizational needs.     Dismissing patient, family, and caregiver concerns tops ECRI’s 2025 list of the most significant threats to patient safety. "A health care system that prioritizes patient voices is one that delivers safer, more efficient, and more compassionate care for all," said Marcus Schabacker, MD, PhD, president and chief executive officer of ECRI.

    We asked 16,005 patients about shared portal
access with care partners... Fewer than 1/2 of patients knew they could share access with care partners. But the study found most care partners who shared access with a patient also were: 1️⃣ Able to log in more 2️⃣ Able to view more lab notes and labs 3️⃣ Able to schedule more visits This research underscores the need for improved strategies to raise awareness and support care partners—especially as patient portals become increasingly vital for managing health information. #PatientEngagement #CarePartners #OpenNotes Read more: https://lnkd.in/gxusbMXY
