“There are ways to foster continuity of care for patients with complex needs without overhauling entire systems or increasing caregivers' workload.” Our Medical Director Jubril Oyeyemi, MD, wrote an op-ed for AJMC - The American Journal of Managed Care on the necessity of continuity of care in emergency departments to curb frequent patients with complex needs. Drawing from the proven results of our Pledge to Connect program and the “Golden Ticket” program he and his colleagues implemented at Virtua Health Mount Holly Hospital, Jubril provides the case for implementing systems that provide recurring ED patients a familiar care provider and the opportunity to truly address their health-related social needs. A longitudinal study published in Nursing Outlook by a team of Camden-based investigators — including our own Director of Research and Evaluation Dawn Wiest, PhD — showed that school-aged Black and Hispanic children reported higher rates of ED visits for #asthma symptoms than White children throughout the Covid pandemic. The new findings were powered by our Health Information Exchange — a robust pool of patient health records, and clinical and demographic data that spans millions of South Jersey residents. With this tool, we're able to uncover disparate outcomes impacting our communities, and inform strategies to achieve equitable health. Read the full findings here:

Beginning in 2023, the Camden Coalition assisted the Vermont Agency of Human Services (AHS) in evaluating, refining, and expanding its implementation of statewide team-based care strategies. Our support of Vermont Team-Based Care (VTBC) across every AHS department resulted in codification and curriculum designed to ensure that collaborative, whole-person care is a consistent and attainable outcome throughout the entire state. We were happy to meet with South Jersey Assemblyman Cody D. Miller and his Legislative Aide, Esther Lendore, earlier today! Assemblyman Miller’s priority to increase affordable housing throughout his district in Camden, Gloucester, and Atlantic counties reflects our own work with the Housing First program. Housing First, our housing supports initiative that provides a stable home plus wraparound health and social services to chronically homeless NJ residents, is at risk of losing state funding in the FY2026 budget. Despite a decade’s worth of evidence showing that our participants maintain their housing and improve their wellness — and at a time when the state’s homelessness rate has risen by 24% — we need to remind our legislators that these programs are well worth the annual investment.
