Healing humanity one patient at a time.
About us
For nearly five decades, Gardner Health Services has provided high-quality, comprehensive healthcare to people of all ages, backgrounds, and life circumstances. In 1967, community members with help from the Stanford Medical School created a small clinic to meet the urgent health needs of local farm workers in the Alviso community. Since that time Gardner has expanded in response to community needs, now providing medical, dental, vision, counseling, and substance abuse services. Gardner is a critical part of the community’s social safety net. In Silicon Valley, one of the country’s wealthiest regions, more than 350,000 individuals struggle daily to meet basic needs — including healthcare — for themselves and their families. Fifteen percent of Santa Clara residents do not qualify for health insurance at all. Each year, 60,000 of these individuals visit Gardner facilities to receive essential medical, dental, vision, counseling, and substance abuse services that, without Gardner, would be out of reach. Gardner believes that quality of service is not commensurate with a patient’s income level and that all people deserve compassionate practitioners, excellent facilities, and world-class care. Mission Statement: Gardner is dedicated to improving the health status of the communities we serve, especially the disenfranchised, disadvantaged and most vulnerable members. Our mission is to provide high quality, comprehensive health care, including prevention and education, early intervention, treatment and advocacy services which are affordable, respectful, culturally, linguistically and age appropriate.

Thank you Center for Health Care Strategies and California Health Care Foundation for the opportunity to be part of a two-year pilot project with Catholic Charities of Santa Clara County to offer housing navigation services and care management for people experiencing or at risk of homelessness.
California is home to more people experiencing homelessness than any other state in the nation. The state’s CalAIM initiative has created new opportunities for homeless service organizations to partner with the health care sector to better align and enhance care delivery for Californians experiencing homelessness. This profile series explores innovative cross-sector partnerships that are working to improve care and service delivery for people experiencing homelessness through CalAIM services. The profiles share insights from pilot teams that participated in CHCS’ Partnerships for Action: California Health Care & Homelessness Learning Collaborative, an initiative supported by the California Health Care Foundation. https://bit.ly/4iq2mnE Zackiya Grant-Knight, Jahira Sterling, MPA, Meryl Schulman
