Today we’re pleased to announce that one of our network providers, Triad Primary Care (TPC), has transitioned to full-service family and primary care following its acquisition of Lake Jeanette Urgent Care in September 2021. TPC operates with a mission to provide personalized, high-quality care with the convenience of same-day appointments, lower costs, and better outcomes for its patients. The practice is led by two board-certified physicians, both with ties to Yale University – Andrew Chow, MD, FACP and Keung Lee MD, PhD. “We’re excited to provide quality primary care and the convenience of same-day appointments to members of our community, but without the high co-pays and traditional episodic approach often associated with urgent care,” commented Dr. Chow. “We’re committed to educating patients and guiding them on achieving optimal health and wellness through continuous, value-based primary care on as-needed or long-term, preventive basis – all at a lower cost for patients, providers and payers.“ As one patient recently commented, "Within moments, I knew I had made the right choice...They are no longer an urgent care facility, but they promise to see their patients using same-day appointments. If I had been using this provider in the past, that one fact would have saved me thousands of dollars spent in a local emergency room. For more information on how providers can reduce hospital readmission rates and proactively treat patients leveraging our innovative technology and services, read today’s press release at https://lnkd.in/eRrdKfKP. #healthcare #primarycare #valuebasedcare #healthinformationexchange
