In honor of American Diabetes Month, I’m proud of organizations like Atrius Health, Kelsey-Seybold Clinic, Reliant Medical Group and Optum Washington for their recent gold-level recognition in the American Heart Association’s Target: Type 2 diabetes program awards. Organizations who received this honor are recognized for their work related to diabetes control and cardiovascular risk factors, hypertension and cholesterol management. Patient health education plays a critical role in the management of Type 2 diabetes. This comprehensive program takes the specialized care team’s longstanding commitment to providing personalized care a step further, integrating high-quality, adult primary care with varied specialty health services to better meet the unique needs of LGBTQ+ patients.
