Angry old guy rant coming… There is a real danger of using non-fusion devices to expand surgical indications…. Today, I saw a 16 yr old female Sanders 7/8, Risser 4+ with a single Xray with 37 degree(my read)/40 degree(initial treating surgeons read) curve. They came for a second opinion and were panicked because they were told that surgery was necessary and that waiting was not ideal. First, a single Xray of a 37 degree curve in a near skeletally mature female is not an indication for surgery. Second, motion sparing surgical interventions do not justify operating on smaller curves that are not progressive, painful, or aesthetically noticeable. The approach taken by the surgeon frustrates me…. In no situation did this curve warrant surgery…. Whether ‘motion sparing’ or not… and it definitely didn’t warrant encouraging the family to have surgery or imply that it was the standard of care. I see this more and more often … where non-fusion options are being used to expand indications for surgery without evidence. 📰 The Lancet: New study shows low back pain is the leading cause of disability around the world 📰 💡 By 2050, a 36.4% rise in low back pain cases is projected, with the highest increases in Asia and Africa.  💡In 2020, 619 million people suffered from lower back pain globally.  💡By 2050, 843 million people are projected to fall into this category.  💡Risk factors are work-related ergonomic factors, obesity, and smoking.  💡Study shows low back pain increases with age and the peak age impacted is 85.  💡Globally, low back pain is more prevalent in women than men in all age groups.  Regions with the highest occurrence of low back pain are Central Europe, Eastern Europe, and Australasia. Increase in low back pain cases will be driven by population growth except in Asia and Latin America, where the main driver will be an aging population. A new study shows low back pain remains the leading cause of disability globally. In 2020, low back pain impacted 619 million individuals. That number is expected to increase in the next three decades to 843 million. Failed back syndrome exists…. but the mechanical complication of non-union is far more common… and this is fixable.
