Forms for submission to the Administrative Committee and other committees
The standards and procedures relative to the preparation and submission of requests to the Administrative Committee and other committees are outlined in Procedure Secretary 20-1. The Critical Path of Approvals outlines every type of decision and establishes where the decisions are made.
Please note that the forms can be completed electronically and then printed out. Please ensure that you have included the necessary documentation with your submission and have made the appropriate number of copies for consideration by the committee. Checklists are available and should be completed for appointments, tenure, promotions, promotions and tenure and academic leaves.
