How to Make an Information Request
All requests must be made in writing by completing an Access Request Form:

If you are making a request for general information, there will be an initial fee of $5. If the total cost of processing your request exceeds $100, you will be provided with a fee estimate before processing begins and you will be required to pay a deposit of 50% of the total processing costs before the University begins to process your request. HOW TO COMPLETE THIS FORM
Your Request:
Please check the appropriate box depending on whether you want access to :

your own personal information,

general records of the University

or to submit a request for correction.
If you are requesting access to or correction of, your own personal information, please indicate the last name appearing on the records if different than name detailed below.
Requestor:
Please enter your last name, first name and middle name. Enter your complete mailing address and your daytime and evening telephone numbers. The University may need to contact you if there are any questions about your request. If you have a fax number or E-mail address where correspondence can be sent, enter them in the spaces provided.
Details of Request:
Please be as specific as possible in describing the records. The more specific your request, the quicker and more accurately it can be answered. If you need more space, please continue your description on a separate sheet of paper and attach it to this request form.
If you are requesting your own personal information, please be sure that you give: your full name; any other names that you have previously used; and any identifying number that relates to the records, such as your employee or student number, or other identification number.
You may examine the records at the University or a copy will be sent to you. Please check the appropriate box.
