This FEops-guided pre-procedural planning session highlights how advanced simulations are used to determine optimal access, device size and placement for LAA occlusion. The case features an 83-year-old male with paroxysmal AF on rivaroxaban, CHADSVASC 4, HAS-BLED 4, recurrent GI bleeding, hypertension, dyslipidaemia and sick sinus syndrome post dual-chamber PPM, with normal LVEF and no significant valvular disease. Neil Fam presents a case-in-a-box with a discussion on emerging technologies during an exciting tricuspid session at #TIOCongress2025, featuring a tricuspid valve replacement using the TRiCares GmbH TOPAZ TC-L system. The 62-year-old male patient, with torrential TR, NYHA III symptoms despite GDMT, hypertension, diabetes, permanent AF, and mild MS, has a TRI-SCORE of 5%. TTVR was chosen due to a large coaptation gap, scallops, and leaflet tethering. CT PDD measured 49mm. Sebastian Iturra presents a TAVR in TAVR/TAVR explant case-in-a-box with live discussion during an aortic session at #TIOCongress2025. The patient is a 76-year-old retired nurse with symptomatic severe AS, mild calcific MS, a history of breast cancer post-chemo radiation, non-obstructive CAD, hypertension, hyperlipidaemia, baseline RBBB and OSA. TAV-in-TAV was not feasible, so the heart team opted for surgery. Following TAVR explant and native leaflet excision, Y-root enlargement was performed. An INSPIRIS 25 valve was implanted, up-sized from an initial sizing of 21. The case highlights the rise in TAVR degeneration and the need for lifetime management strategies supported by careful planning.
