  1. Activate immediate rescue and emergency response
  2. Notify all relevant HSE and emergency teams without delay
  3. Record and categorize all incident details accurately
  4. Conduct Root Cause Analysis (RCA) and apply the 5 Whys method
  5. Repair affected areas and initiate preventive measures
  6. Document transparently for audits, reports, and learning
