Comprehensive User Experience Design Enhances Patient Engagement to Improve Outcomes

Traditionally, the development of drug delivery devices focused primarily on are safety and efficacy. However, a therapy's success also depends on patient adherence to prescribed regimens. Adherence rates are significantly influenced by additional factors such as ease of use, comfort, and convenience. These factors are integral components of the overall patient experience.

The importance of the patient experience is increasingly recognized by the industry, reflected most recently in the FDA’s Patient-Focused Drug Development Guidance Series. Emphasizing patient perspectives throughout the drug delivery device development process can enhance adherence and the overall effectiveness of therapies. This evolving focus underscores the benefits of centering drug and device development around patients, benefiting both them and healthcare outcomes.

At Insight by Nemera (Insight), we leverage our expertise in User Experience Design to create positive user experiences with both physical devices and digital health solutions. To do so, we address several engagement factors, including perceived usefulness and emotional attachment, through behavioral, visceral, and reflective design.

Perceived usefulness is essential to product adoption and continued use. Although the relevance of a prescribed device may seem self-evident, it is not always enough to guarantee adoption. To enhance a device’s relevance, we employ a comprehensive understanding of current conditions, informed by the Voice of the Patient (VoP) with an awareness of emerging and future possibilities, to develop functional solutions that meet real needs, both articulated and unarticulated. Effective design ensures that the device not only meets users' needs but also clearly communicates its value and efficiency in solving specific problems.

However, usefulness alone does not ensure a positive user experience. A user’s emotional state profoundly influences their interaction with a device, as emotions can affect cognition. This is particularly relevant in healthcare: users anxious about their condition or therapy (e.g., fear of needles) are less likely to complete their treatment. While designers cannot completely control users’ emotions, they can influence them in several ways:

Visceral Design: This aspect leverages aesthetic and emotional impact. Users’ first impressions, based on visual and physical elements like color, texture, and shape, evoke feelings such as friendliness or seriousness. Effective visceral design shapes positive expectations even before interaction.

Reflective Design: This aspect addresses how a device impacts users' self-image. Some devices, such as defibrillators, should have a distinctly medical appearance to be recognizable or inspire confidence. Conversely, reducing the medical look can diminish cultural stigmas, as seen with insulin pens. Visual and physical/ digital characteristics should align with established mental models to enhance, rather than harm, a user's self-perception.

Behavioral Design: This aspect focuses on comprehension and usability as it is essential that users can both understand and effectively use a device. Misunderstandings and poor usability can lead to frustration and error, ultimately resulting in non-use. Effective behavioral design communicates proper use and accommodates physical or cognitive limitations, especially for therapies targeting those limitations.

Implementing Principles into Practice to Develop Holistic Experiences

Integrating behavioral, visceral, and reflective design principles into devices, instructions, packaging, and digital health assets significantly enhances patient engagement and outcomes. This comprehensive approach ensures that each touchpoint surrounding a drug delivery device is effectively user-centered to provide superior healthcare experiences.

Device Design: User-friendly devices facilitate adherence through intuitive controls and appropriate ergonomics. Enhancing the overall experience with a suitable look and feel reduces negative associations or stigmas, thereby improving user perception and engagement.

Instructions for Use: Clear, concise, and understandable instructions, whether paper or digital, are crucial. Utilizing simple language, diagrams, and appealing layouts instills confidence and enables users to handle the device independently.

Packaging: Packaging should also be designed for ease of use, featuring clear instructions and intuitive pull tabs or tear strips. High-quality, visually appealing packaging creates a positive first impression and should align with societal values, such as the use of eco-friendly materials.

Digital Health Assets: Digital supports, such as apps and online resources, play an essential role in managing drug delivery and chronic diseases. Effective design ensures accessible navigation, clear guidance and other information, and visual appeal. Apps like MyTherapy help users track adherence while platforms like Medisafe assist in managing chronic conditions, positively reinforcing users' empowerment and motivation.
