As a company that is at risk for an entire population, regardless of if we are able to find or engage all the members in the population, it is very difficult to explain "what we do" to engage and help people. This member story is the best I have seen at explaining our care model. None of this story would be possible under the fee for service reimbursement model as we would not know who ALL the members are who are diagnosed and untreated, nor is there a CPT code for most of the activities described, and therefore there is no reimbursement model to pay for those activities. And since clinical and financial outcomes drive our entire economic model, we can't give up on people ever. We have to care deeply if people have providers, if they make it to their appointments, if they have housing, if they have food, if they are in the ER, if they have a good discharge plan, if they fill their prescriptions, if they are in crisis, and on and on and on. This is the "purest" healthcare I have seen as it relates to having the flexibility and the incentive to do what we think the patient needs and what we think works to improve care and reduce avoidable costs. This is the most challenging work, rewarding work, and fun work I have ever done.
