At The Medical and Diagnostic Care Group , our commitment to providing bespoke solutions for each NHS England Trust is at the heart of what we do. Over the past two weeks, we've been on the ground with our partners, visiting Trusts sites to understand their specific needs, ensuring our services align perfectly with their existing operations amd the implementation is smooth and carefully planned. This weekend marks the launch of a new contract across three acute hospitals within a Trust, where we’ve carefully tailored our approach to enhance efficiency by building on the services they already have in place. Looking ahead to next weekend, we’re preparing for another contract launch that spans a wide range of care—from outpatient services to surgical procedures and the management of a full-day unit, including admissions, procedures, and patient discharges. Our goal is to boost service capacity, allowing us to treat more patients while supporting the Trust in reducing waiting times. Surging Hospital Prices Are Helping Keep Inflation High https://lnkd.in/e8Nzxqif Interesting article in the WSJ today connecting rising inpatient care costs to the stickiness of overall US inflation. “Hospital prices specifically jumped 7.7% last month from a year ago, the highest increase in any month since October 2010, the Labor Department said Wednesday.“ Stating it’s a multifaceted challenge is putting it lightly. I’m proud to work at CCX to support our hospital partners. Through tech, clinical, and network enablement our org supports acute care teams to efficiently and effectively discharge patients safely HOME. Multiple analysis / studies point to solid discharge planning as a critical component to reduce readmissions and improve quality. SNFs are accepting patients of high acquity with higher risk of readmossion to acute care due to. - More complex patients with higher CMI are being admitted - Under staffing - Clinicians in these facilities are less trained to take care of high acquity patients. - Poor coordination of care from acute to post acute care. - Lack of proper discharge information due to lack of integration between different EMR in acute care and post acute care. - Poor afterhour physician staffing and communication.
