During our last conference, the following question was asked several times: "What type of anaesthetic is recommended before applying DEBRICHEM®, and how long should it be used?" 🌟ANSWER: In most patients an anaesthetic cream is sufficient. The most commonly used cream is 5% EMLA or 5% lidocaine. Other forms of anaesthesia that can be used are local infiltration with lidocaine (or similar) and truncal, spinal, or general anaesthesia. The method to be chosen depends on the general clinical status of the patient, his/her tolerance to pain, the wound characteristics etc. NOTE: every patient is experiencing pain differently. "Symptom or Side Effect?" – A Game Every Healthcare Professional Should Play In geriatric care, spotting the difference between a new symptom and a drug side effect can be a real game-changer. Consider this recent case: A patient was prescribed tamsulosin to manage urinary symptoms of BPH. Soon after, they began experiencing persistent back pain—a lesser-known side effect of tamsulosin. This pain affected their mobility, leading them to seek pain medications and physical therapy. Instead of relief, these interventions added new side effect risk and complexity to their daily routine. Fortunately, a trial discontinuation of tamsulosin not only relieved the back pain but also restored the resident's mobility. Pain medications and therapy were were no longer needed. This experience highlights the importance of playing "Symptom or Side Effect?" every time a new issue pops up. Personalized treatments are transforming how care is delivered in the Schroeder Pain Assessment and Rehabilitation Research Centre (SPARC). “In the current model, the patient bounces from treatment to treatment based on what the family doctor thinks works in general,” says Dr. Dinesh Kumbhare, director of the SPARC pain clinic and the Schroeder Chair in Pain Assessment and Rehabilitation at Toronto Rehabilitation Institute. In the SPARC clinic, the interdisciplinary team first develops a personalized treatment plan for pain by grouping people into subcategories based on the cause of their pain and looking at comorbidities simultaneously. This system makes care much more effective and efficient by allowing clinicians to narrow in on the treatment that will work the best right away instead of through trial and error. “Clinicians provide the treatment promptly, which is a better experience for the patient and the healthcare professional as well,” says Dr. Kumbhare.
