Seeing long-term results for knee OA with PRP are great with 10B+ platelets but also here is multi-level DDD with facet OA - treated in 2019 -- presents today for repeat treatment: 5+ years of great benefit with 5-10B platelets into facets; 1-3B into epidural space (bilat S1, L4-5: 10cc total epidurally); ligaments Prolo; PPP multifidus -- Full FSU (extradiscal) happy friday !

This is fantastic - 5 years! What helped drive the decision NOT to do the discs and to use PRP over BMAC?

Jason, I am another happy customer of L4/5 and L5/S1 FSU with PRP, performed by Dr. John Pitts. My sole treatment was 51/2 years ago now and I remain (on my bad days) 80% better than my baseline. I now perform the same technique on my patients in Houston with similarly-excellent results. I reserve intradiscal injections for more narrow indications.

But…a complicated question for you, coming from a 59 y.o. guy who’s L5/S1 disc space has collapsed —slowly and relatively asymptotically— over the past 20 years; At what point along the way do you think we as interventional spine physicians are obliged to counsel our patients that these discs want to degenerate (and will) as time goes on, leading to problems down the road like: —Ligamentum flavum buckling —Facet joint hypertrophy —symptomatic canal/LR/NF narrowing —neurogenic claudication Is the safety data of disc-specific treatments (LR-PRP/CE MSCs/Discseel/etc) strong enough yet to advise our patients to consider using them as part of a preventative maintenance regimen? You are da man! Thanks 🙏
