Patients in rural counties often face considerable drive times and wait lists of 6 months or more to see a specialist. Disparities in Post-Hospitalization Care: The Road to Recovery is Uneven A study recently published in *Annals of Internal Medicine* highlights a crucial issue: disparities in post-hospitalization care are leaving some patients behind. While we often focus on the quality of care inside the hospital, the care after discharge is equally critical for recovery—and this is where many patients from marginalized communities face significant barriers. The study reveals that patients in underserved populations are more likely to experience poor transitions from hospital to home, resulting in higher rates of readmission and adverse outcomes. Factors such as limited access to follow-up care, cost of medications, social and family support, and understanding of discharge instructions are contributing to these disparities. Equitable healthcare doesn’t end when a patient leaves the hospital. We must prioritize improving post-discharge care and ensure patients are supported in their recovery journey. This has hit my feed a couple of times since pub. I get frustrated every time I read it. Data highlights the correlation between Provider:Patient face time and inappropriate prescribing. The knee jerk reaction is to blame the provider but that doesn’t address the real issue: ‘By a recent estimate, primary care clinicians would require 27 hours per day to provide all guideline-recommended preventive, chronic disease, and acute care to a typical patient panel.’ It’s time to shift the dialogue from what Providers can do, to how we can enable them to do what they must. 💡👇 When we talk to providers about the value #Nomo can help bring to their practice, we hear: 1.
