🚨 Process Incident Summary: Moerdijk Hydrogenation Reactor Explosion 🚨 Incident Title: Hydrogenation Reactor Catastrophic Failure Incident Type: Explosion and Fire Date: 3rd June 2014 Location: Moerdijk, Netherlands Summary: An exothermic runaway reaction during the startup of a styrene monomer and propylene oxide (MSPO) manufacturing plant led to two explosions and a major fire. The incident occurred due to manual adjustments during the heat-up phase, resulting in overpressure and subsequent failure of the reactor. Consequences: • Fatalities: 0 • Injuries: 2 • Financial Impact: Unknown Root Causes: 1. Inadequate communication and management of change regarding a new catalyst formulation. 2. Inadequate reactor instrumentation and design flaws. 3. Failure to reset critical gas venting systems. 4. Lack of thorough investigation into a similar incident at a sister plant. Lessons Learned: 1. Utilize quantitative reaction hazard assessment data for safeguard design. 2. Conduct rigorous management of change reviews for any process alterations. 3. Clearly identify safety-critical steps in operating procedures. 4. Ensure control systems provide stable process control during startup and transient conditions.
