The Intensive Mental Health Provider will Coordinate and link the child and family to supports, resources and services within their community to ensure that the child and family have the needed support to maintain the child in the home.
Responsibilities
Conduct and maintain all visit requirements with the Child, Family and Service Providers.
Provide Skill Building services to clients as needed and in accordance with their service plan.
Provide on-going assessment and documentation of child and family's strengths and needs, progress towards achieving goals and efficacy of delivered services.
Coordinate and monitor Family Support and Youth Peer Advocacy Services, including attending monthly team meetings.
Provide linkage and referrals to additional services and supports.
Complete and Submit all reports, progress notes, service plans and any other written work on time and in compliance with Agency, County and State Regulatory Standards.
Keep client files up to date and properly filed in accordance to program and OMH guidelines.
Additional responsibilities as assigned.
Qualifications
Bachelor's Degree in Human Services related field Minimum of two years of experience providing direct services to children within the Children's Services System
Experience with children with emotional disorders preferred.
Excellent verbal and written communication skills.
Intermediate Computer Skills with Microsoft Office
Valid and clean driver's license

Pay: $43,999/Annually
Schedule: Full Time, 35 hours per week
