In a randomized trial in over 12,800 patients with type 2 diabetes, hypertension, and other cardiovascular risk factors, patients assigned to a target systolic blood pressure (SBP) of <120 mmHg had a lower incidence of a cardiovascular composite of nonfatal stroke, nonfatal myocardial infarction, heart failure, and cardiovascular death at a median of 4.2 years, compared with patients assigned to a target SBP of <140 mmHg. These data, combined with previous studies, support a strong recommendation for intensive BP lowering (e.g., SBP <125 mmHg using standardized methods) in patients with diabetes mellitus. A new study of over 31,000 patients with rheumatoid arthritis (RA) demonstrates that those with mild disease activity had an additional decline in their annual estimated glomerular filtration rate (eGFR) of 0.1 mL/min/1.73 m2 compared with patients with RA in remission. Patients with moderate or severe disease activity had an additional annual eGFR decline of 0.2 mL/min/1.73 m2. This study implies that tight control of disease activity may help preserve kidney function among patients with RA. Low-dose lithium does not appear to benefit patients with long COVID-associated neurocognitive symptoms. In a placebo-controlled randomized trial in 52 patients with at least four weeks of neurocognitive symptoms following COVID-19 infection, three weeks of lithium aspartate (10 to 15 mg daily) failed to improve fatigue or cognitive dysfunction scores.
