About us

Community Care Plan (CCP), 'the health plan with a heart,' was introduced in 2000 as South Florida Community Care Network (SFCCN), the first safety-net hospital-owned Provider Service Network. CCP serves 150,000 lives enrolled in Medicaid, Florida Healthy Kids, uninsured programs, and self-insured employee health plans. CCP is owned by Memorial Healthcare System and Broward Health. Community Care Plan maintains accreditation and has earned Health Equity accreditation for its Medicaid Health Plan by the National Committee for Quality Assurance (NCQA). CCP is also fully accredited by the Accreditation Association for Ambulatory Health Care (AAAHC) as a Health Plan.   • Last Sunday, the Community Care Plan team laced up their sneakers and walked with heart at the Broward Heart Walk at Nova Southeastern University! Together, our health champions raised over $30,000 in support of the American Heart Association’s mission to be a relentless force for a world of longer, healthier lives. Why We Walk: ❤️ 9 out of 10 people who suffer cardiac arrest outside the hospital don’t survive—often because bystander CPR wasn’t performed. ❤️ We can change this. With more people trained in CPR, we can turn bystanders into lifesavers. ❤️ Every step, every dollar, and every heartbeat counts.   💪 Thank you to our incredible CCP team, donors, and walkers for making a lifesaving impact!   • Community Care Plan and Unite Us Partner to Bridge Healthcare and Social Services, Advancing Health Equity Across Florida🌴💙 CCP is proud to partner with Unite Us, the nation’s trusted technology leader in social care transformation. Together, we’re making it easier for our members to access the critical social resources they need—because health care goes beyond the doctor’s office. Through Unite Florida, CCP can facilitate closed-loop referrals to connect individuals with community-based organizations (CBOs) that provide essential services, from housing support to food assistance and beyond.

  • CCPeeps spent Saturday celebrating wellness at the South Dade Health & Wellness Fair. The community event provided free health education and health screenings at Homestead Hospital. 🏥 Volunteering at this event is another step we’re taking towards breaking down barriers to high-quality care in our new regions. 💙🌴

  • If you are a BA Provider, rest assured that the transition to the new Medicaid contract has been carefully planned with processes in place to support you and your patients. The Continuity of Care (COC) period extends up to 90 days from the time the member enrolls with CCP. Our commitment is clear: 85% of claims will be paid within seven calendar days, and standard authorizations will be processed within five. We value our partnership and remain dedicated to supporting you in providing quality care to our community!
