Congratulations to Christine Magnuson, clinical specialist, who represented Luminis Health at the American Case Management Association (ACMA) National Conference in Denver—presenting on our impactful Mobile Integrated Health (MIH) initiative. Christine’s poster was selected as one of just 26 presentations from hundreds of submissions nationwide, a powerful recognition of the innovative and patient-centered work happening across Luminis Health. Our MIH model is rooted in a simple, but powerful truth: no one heals best in a hospital bed. The program helps patients get home sooner, with the right supports in place—saving nearly 5,000 hospital days and reducing readmissions by over 6.6%. Behind every number is a patient who had the chance to recover in the comfort of home, thanks to the seamless collaboration across our care teams, from case management and pharmacy to Procare Ambulance and Home Health. This initiative is a true embodiment of our RISE values: Respect, Inclusion, Service and Excellence — and a reflection of what’s possible when healthcare meets patients where they are.

Just returned from the American Case Management Association (ACMA) National Conference in Denver feeling inspired, energized, and proud to be part of such an impactful community! I had the incredible opportunity to present our Mobile Integrated Health (MIH) initiative at Luminis Health as a poster presenter, and it was an honor to showcase the innovative work our team is doing to bridge care gaps and improve patient outcomes. To date, our MIH program has saved nearly 5,000 bed days and achieved a 6.6% lower-than-expected readmission rate—real impact we’re proud of. It was also a joy to connect in person with so many of the partners and support teams who help us do what we do best—whether through technology solutions, staffing strategies, or sharing best practices.
