Postpartum hemorrhage (PPH) is the leading cause of maternal mortality worldwide, resulting in about 70,000 deaths each year, most in low- and middle-income countries. In the Democratic Republic of Congo, Guinea, India, Kenya, Nigeria, and Zambia, the Jhpiego-led AMPLI-PPHI project is working to end deaths due to PPH. The project provides training to community health workers and health facility staff to recognize and manage PPH through the use of quality-assured PPH drugs as part of the E-MOTIVE approach. For decades, the default response to health workforce performance gaps in LMICs has been "more training." However, evidence from a multi-country analysis suggests it's time to ask: training... plus what else? Standalone classroom-based training and traditional supervision remain the norm, despite limited evidence of their effectiveness. Let’s move beyond a one-size-fits-all training approach. A more effective strategy combines digital learning, workplace support, and data-driven interventions—tailored to real-world contexts. Midwives in low-and middle-income countries face many challenges, from broken equipment and limited supplies to staff shortages and inadequate infrastructure. They also regularly deal with trauma and stress. Club Courage, a collaboration between Jhpiego and Think Place, helps midwives to address these issues. Club Courage meetings have both improved relationships and communication among participants and helped them to understand and address their own feelings. Said one midwife participant in Mali, “If midwives feel supported, they will be better at providing respectful and empathetic quality care to the patients.” Health workers are the first line of prevention and the principal providers of health care services.
