💡 AI in Clinical Trials: Are We Repeating the Biases of the Past? AI has the potential to revolutionize clinical research, but as Lisa Moneymaker warned at ING2024, it also has the power to amplify biases embedded in historical data. 📢 “Our data tells a big, complicated history. We have a moment to ensure that AI does not simply copy what our grandfathers knew— but that it learns from their blind spots to define a better future.” 🪢 The Bias Feedback Loop: 📍 AI doesn’t just reflect bias—it amplifies it. If past data lacks patient diversity, AI models will double down on those gaps. 📍 Unchecked bias in AI can shape real-world decisions. Skewed algorithms may influence everything from trial eligibility to protocol design—excluding the very patients we need to reach. 📍 Historical trial data isn’t neutral. Marginalized patients have often been underrepresented. AI trained on this data risks repeating underrepresentation. 🎯 So, what’s the solution? Lisa made it clear: we must have diverse voices at the table. People training AI for clinical research must actively mitigate bias. If we get this right, AI can make trials more inclusive. 💡 Fixing the Enrollment Bottleneck in Clinical Trials A recent Applied Clinical Trials Magazine interview highlights a persistent challenge—patients aren’t enrolling in trials because their physicians aren’t informing them about trial options. But is awareness the only issue, or is trust just as important? 📢 Laura O'Donnell put it plainly at ING2024: "We fail to create trust as an industry and to have meaningful engagement with the communities that we're trying to serve." Faster trial enrollment isn’t only about access—it’s about confidence. If patients don’t trust the system, they won’t engage in trials. Enrollment Challenges: 📍 Lack of awareness: Many patients rely on their doctors for trial information, yet the physician communication gap leaves them in the dark. 📍 Marketing failures: Many trials aren’t marketed effectively—not to sites, not to patients, and not even to their physicians. 📍 Rigid trial designs: When trials don’t fit into real patients’ lives, participation suffers. 📍 Trust & transparency gaps: Patients don’t just need to know a trial exists—they need confidence in the process and trust in its benefits. 🚀 We can (and must) do better. 💡 AI in Clinical Trials: Is Our Approach to Consent Falling Behind? Informed consent isn’t just a checkbox—it’s a patient’s fundamental right to understand how they’re participating in research. But as AI becomes more integrated into clinical trials (and everything else), our consent processes must evolve to keep up. Here are some key questions the industry should address: 📍 Are patients fully informed when AI influences trial participation or decision-making? 📍 Do language barriers or overly technical explanations inhibit true understanding and consent? 📍 How do we balance AI’s efficiency with human oversight and ethical transparency? 📢 At ING2024, Jonathan Norman highlighted major localization gaps in clinical trials.
