As the leader of our Family Medicine Clinics, Outpatient Clinic, and Surgery Department, Monica plays a vital role in ensuring high-quality, compassionate care for our patients. Balancing her professional responsibilities with her personal life as a wife and mother (including to a busy preschooler!) while completing a rigorous doctoral program is truly inspiring.   • Welcome, Brigette Cowan, RN, BSN, Director of Nursing to Holton Community Hospital. Holton Community Hospital is thrilled to welcome Brigette Cowan, RN, BSN, as our new Director of Nursing. Brigette brings over 20 years of comprehensive experience in emergency and critical care nursing, leadership, and clinical supervision. She earned her Bachelor of Science in Nursing from Washburn University, which laid the foundation for her distinguished career in healthcare. Most recently, Brigette held a leadership role in perioperative services for the past two years and, before that, served as the Director of Emergency and Critical Care Services at The University of Kansas Health System St. Francis Campus. In this role, she managed clinical operations, staff development, and quality assurance, ensuring the delivery of safe, effective, and compassionate care for patients with critical needs. Brigette’s journey in healthcare began with multiple roles within the Emergency Department at St. Francis Health. She served as a Charge Nurse for over 12 years, where she supervised patient flow, staffing, and quality of care. She also played an instrumental role in initiatives focused on Joint Commission readiness, stroke certification, and chest pain accreditation. Her expertise extends to nursing education, clinical supervision, and maintaining high patient satisfaction. With a strong background in emergency nursing and a commitment to clinical excellence, Brigette is excited to lead the nursing team at Holton Community Hospital.

  • HCH Announces the Closure of it's Home Health & Hospice Services It is with great sadness that Holton Community Hospital (HCH) announces the closure of our award-winning Home Health & Hospice department, effective December 31, 2024. After many years of subsidizing this vital service, the HCH Board of Directors made the difficult decision to close due to significant changes in healthcare payment models, including the expansion of Medicare replacement plans, and the growing number of competing agencies in our area. We deeply appreciate the dedication of our staff and the trust of our community. Our priority is ensuring a smooth transition for all patients.   • 1. What is a CHNA? A Community Health Needs Assessment (CHNA) is a systematic process that hospitals conduct every three years to identify and assess the health needs of the community they serve. The CHNA involves gathering data on health outcomes, risk factors, and social determinants of health, as well as collecting input from community members and local health experts. The purpose of the CHNA is to guide hospitals in prioritizing health issues and developing strategies to address them, ultimately improving the overall health of the community. 2. What areas are included in the CHNA requirement? The CHNA requirement includes several key areas: - Community Engagement: Involving a wide range of stakeholders, including public health officials, community leaders, and representatives of underserved populations, to ensure diverse input. - Health Data Collection: Gathering and analyzing data on various health indicators such as disease prevalence, mortality rates, health behaviors, and access to healthcare services. - Identification of Health Priorities: Based on the data and community input, the hospital identifies and prioritizes the most pressing health needs in the community. - Implementation Strategy: Developing a plan of action to address the prioritized health needs, including specific goals, resources, and evaluation measures. - Public Accessibility: Making the CHNA report and the hospital’s implementation strategy available to the public, typically through the hospital’s website. 3. How does the hospital define its community? Holton Community Hospital defines its community as the geographic area and population it serves, which includes 35 mile radius from HCH. The community is not just limited to patients who use the hospital's services but also includes those who live, work, and engage in the area. This broader definition allows the hospital to consider a wide range of health needs and to focus on improving the health and well-being of all residents, especially vulnerable populations. 4. What questions should a CHNA answer for a hospital? A CHNA should answer several critical questions: - What are the most significant health issues affecting our community? - Which populations are most impacted by these health issues? - What are the root causes or contributing factors to these health issues? - What resources are currently available in the community to address these health needs? - Where are the gaps in services or resources that need to be addressed? - What actions can the hospital take to improve community health outcomes? - How can the hospital collaborate with other organizations to address these needs effectively?
