Celebrating Excellence: GLIN IPA honors our Care Management team for their remarkable Care Management Wins! Dedication to Recovery: Supporting Sobriety & Mental Health Julia, LMSW, Social Work Care Manager, helped a patient in the TCM program who was drinking heavily and searching for a new counselor after several unsuccessful attempts at mental health linkages. By helping him navigate his insurance, narrow down his preferences, and set up an appointment with a psychiatric NP and mental health office, Julia connected him to a provider he felt comfortable with. By the end of the program, the patient celebrated one month of sobriety. Holistic Patient Support: Addressing Medical & Social Needs Jennifer, MS, BSN, RN, Director of Care Management, quickly identified a patient’s medication needs and coordinated with the PCP to ensure prescriptions were filled.
