Time for celebration! 💚Our first ESCH-R paper has been published: “Accelerating the transition from a linear to a circular healthcare sector: ESCH-R: study design and methodology.” Due to the significant environmental impact of healthcare, there is an urgent need to accelerate its circular transition. In this publication, the authors provide an overview of the ESCH-R project study design and methodology for accelerating the transition from a linear to a circular healthcare sector through the development and implementation of circular interventions in the Netherlands. Using a transdisciplinary approach, the ESCH-R consortium will apply the 10-R ladder framework for a circular economy to hospitals. Methods are presented to analyze current clinical practices, policies and requirements for sustainable behavioral change, from material flows and operations to policy and regulations. The authors describe methods for the development of circular interventions, including business models, contract templates, and product redesigns. The described study design and methods can be used by other hospital (settings) to identify environmental hotspots for circular interventions in their own healthcare practice and for the cross-transfer of knowledge and anticipated challenges in implementing circular strategies. Ultimately, the ESCH-R project will deliver innovative, scalable approaches for hospitals to reduce procurement of raw materials, retain value of medical products, and reduce waste streams, CO2 emissions and pollution. Today is Global Recycling Day! How are you contributing towards a more circular hospital? The ESCH-R consortium focuses on adopting circular interventions in hospitals to reduce the ecological footprint of the healthcare sector in terms of CO₂ emissions, material extraction, and waste. Our PhD’s, postdocs and researchers Jasper Klasen, Ayşegül ÖZÇELİK, Changlung Tsai, Yuanyuan (Alice) W., Jacopo Parma, Joek van der Zwaan, Paula Goetz, Yifan Yang, Shalvi Thakur, Heidi Annala and Sanne Jansen are currently working on their research project, each in their own track, all contributing to answering the overarching research question: “How can hospitals move away from single use medical consumables and make the transition towards circularity?” The focus within ESCH-R is to identify key environmental impact hotspots related to medical consumables used in currently six departments: the intensive care units and intervention cardiology and obstetric departments of the Erasmus MC, and the ophthalmology, urology and nephrology departments in UMC Utrecht. Within these two hospitals, we have established two Living Labs, where medical devices and interventions are tested, developed, and validated by our PhD’s, postdocs and researchers based on the 10-Rs framework for circularity. Let’s Refuse, Rethink, Reduce, Re-use, Repair, Refurbish, Remanufacture, Repurpose, Recycle, and Recover! Nicole Hunfeld, Principal Investigator for the ESCH-R Project, has been dedicated to circularity and sustainability in healthcare for years. With her expertise and passion for circular healthcare, she fosters innovative solutions and collaborations within the ESCH-R consortium to reduce the environmental impact of the healthcare sector. Did you know that an ICU patient at Erasmus MC generates an average of 17 kg of waste per day? Most of this waste is non-recyclable, including disposable gloves and medical packaging. To change this, Nicole Hunfeld, Chief Sustainability Officer and hospital pharmacist at the Intensive Care Unit of Erasmus MC, launched the Green Intensive Care project a few years ago. In collaboration with TU Delft and Erasmus Universiteit Rotterdam under Convergence | TU Delft-Erasmus MC-EUR, the project examines the most polluting waste streams, explores circular and sustainable alternatives, and works towards reducing the CO₂ footprint of the Dutch healthcare sector. ♻️ Circular transition in healthcare, here’s Rotterdam’s approach By 2030, circularity will be a way of life, paving the path to a fully circular economy by 2050 in Rotterdam. This means re-imagining how resources are used across industries, from construction to healthcare. One example is rethinking waste in the city’s hospitals, where valuable materials are often used once and then discarded. At Erasmus MC, every ICU patient generates an average of 17kg of waste per day, most of it is non-recyclable, including disposable gloves and medical packaging. That’s why Nicole Hunfeld, Chief Sustainability Officer and Hospital Pharmacist of the Intensive Care Unit at Erasmus Medical Center, initiated the Green Intensive Care project a few years ago. Through collaboration with Delft University of Technology and Erasmus University Rotterdam under the Convergence | TU Delft-Erasmus MC-EUR, the project is identifying polluting waste streams, exploring sustainable and circular alternatives, and working with experts across disciplines to minimize the CO2 footprint in Dutch healthcare. In a circular hospital ecosystem, what value do you bring to the table? ✅ On Tuesday, February 11th, the second co-creation session of workpackage 4 took place at Microlab | (Flex)kantoren, maker space, evenementen & coworking in Rotterdam. With all stakeholders in the room – ranging from raw materials suppliers, Original Equipment Manufacturers (OEM), to waste processors – we discussed what the future of a circular hospital ecosystem would look like, and how value exchanges would be different compared to the current ecosystem. With this step, we aim to identify leverage points, as well as potential frictions that may enable or hinder the transition to new business models and purchasing strategies in that circular hospital ecosystem. On January 31st, the first ESCH-R Science Day was held at the UMC Utrecht! How far have we come with our research? WP leaders and their PhD candidates shared insights on circular innovations, hospital procurement strategies, behavioral change within hospitals, and the role of Living Labs in creating real-world impact! The event was chaired by our PI Nicole Hunfeld (Erasmus MC) and project board member Albert Wagelmans (Erasmus Universiteit Rotterdam). The Science Day programme also included an interesting keynote by Dr Ravi Ravindran, Director Quality of the Aravind Eye Hospital - India in Madurai, India. The Aravind hospital group, which performs 450.000 cataract surgeries per year, produces only 5% of the amount of waste that is produced in hospitals in the Western world. Inspiring insights we in The Netherlands can learn from! Thank you to all our workpackage leaders and our PhD candidates for providing an update on their research and resourceful insights! And thank you to everyone who joined Science Day! On January 27th, Gupta Strategists organized a solution-based thinking workshop for PhDs and Postdocs in the ESCH-R consortium. Designed in collaboration with Gupta Strategists, Niels Hagenaars, Lisa Vogelpoel, Anouk van der Schot, and TU Delft professors Jan Carel Diehl and Jotte De Koning, the workshop focused on equipping participants with structured strategic thinking tools. During a previous visit to Philips, PhD students from the ESCH-R consortium, along with other consortium members, participated in a small assignment on the Pulse Oximeter. A key takeaway was the variety of perspectives on how PhD students would approach their research on this topic. Inspired by this, Jan-Carel Diehl and Jotte de Koning, together with Gupta Strategists, organized the first training day, focusing on skill development, team building, and the Pulse Oximeter as the central theme. In each episode, hosted by Nicole Hunfeld (ESCH-R Coordinator) and Dimitry (DIM) Vleugel, we bring together sustainability and circularity experts from industry leaders such as Wittenburg Group, alongside medical professionals driving change in healthcare. Our second episode features Anne Looije-Traa and @ Priscilla Lips from Wittenburg Group, discussing the importance of the partnership between ESCH-R and Wittenburg Group and how they reinforce each other. Priscilla: “Since the start of the ESCH-R project, we have more frequently received insights from hospitals. And with that, we’re gaining a much better understanding of what’s really going on in hospitals. What do healthcare providers need? What is important to them? What actually happens to waste in hospitals? All that knowledge and information really helps us in the discussions with our customers and to align on how we can take the next step together.” And last but not least, at the end of the podcast, Anne and Priscilla posed key questions to our next podcast guest Philips: "How does Philips specifically apply EcoDesign in the development of new medical equipment? How do they determine and prioritize the sustainability criteria relevant to specific product groups? Additionally, we are curious about how collaboration with hospitals within the ESCH-R project could contribute to this!" 🎧 Tune in to discover the steps being taken toward a circular healthcare system, with contributions from all parts of the circular value chain — from manufacturers to hospitals and waste processors.
