Postpartum Support. Trusted Relationships. Research in Action. This video from the University of Pennsylvania National Center for Health in Public Housing explores how our approach to home visits and midwifery care are transforming maternal health. The research is clear: when care extends beyond the birth room and into the home, families thrive. Midwives and community health workers build trust, identify concerns early, and help new parents feel seen and supported. At Community of Hope, we believe in these models. We make sure that they are patient-centered. We live them out. And we’re proud to be part of the national conversation driving this care forward—grounded in both compassion and data. With hope, one can accomplish anything. At Community of Hope, we believe every family deserves quality healthcare, stable housing and the opportunity to thrive. For more than 40 years, we have been committed to improving health, ending homelessness and partnering with communities to make DC more equitable. With the support of our community of donors, volunteers, and partners, we provide services that empower families to achieve good health, a stable home and lasting hope. After spending last year of listening deeply to patients, clients, staff, board members, and community leaders, we are proud to introduce Community of Hope’s 2025–2029 Strategic Plan: Health. Housing. Hope. Approved by our Board in December 2024, this plan affirms and tweaks our mission. We’ve broadened our mission to reflect our role in ending homelessness for individuals. We are now highlighting how we center client voice and our work to address policy change in DC. Guided by our values, this plan will serve as a roadmap to deepen our impact, advance partnerships, and deliver quality care. Our strategic plan—along with the support and partnership of you, our community—will help us lead with hope, no matter what comes our way. Want to read more? Marriott Foundation, and The Bedford Falls Foundation have committed a total of $5.5 million to ensure the sustainability of our innovative model. What does hope look like? For Shannelle, it meant finding stability, support, and a fresh start. As a single mother of three, leaving an abusive relationship meant carrying the financial burden alone. Struggling with housing instability and mental health challenges, she wasn’t sure where to turn—until she found Community of Hope. Through our Rapid Re-Housing Program, Shannelle secured a safe home, received dedicated case management support, and accessed mental health resources for herself and her son. With stability came opportunity—she rebuilt her credit, saved money, and is now moving into a home of her own. 💛 “Hope means taking the pen and rewriting your story… because sometimes, what feels like the end is just a new beginning.” Read her full journey here: https://ow.ly/TZmk50VasAE #HopeInDC #CommunityImpact #AffordableHousing #FamilyStability #SocialWork
