We are thrilled to share our latest publication in Neurospine investigating the biomechanical characteristics of sacroiliac fusion, in a concurrently-controlled study design with respect to joint preparation, implant placement, fixation, long-term durability, and bone-implant interface integrity. The team at Medical Device Development (Richard O. Raji, Sarah Mayer, Joshua Tandio, Jeremi L.) has developed validated in-situ cadaveric and in-silico computational models for the comprehensive biomechanical evaluation of musculoskeletal devices. These models are capable of simulating and analyzing complex anatomy, configurations, and loading conditions with a high level of fidelity, accuracy, and repeatability. In this study, we applied our methods to pioneer the first FDA-approved in-situ biomechanical evaluation of a sacroiliac joint fusion implant. 🔑 Key Findings: 1️⃣ Joint Preparation: One posterolateral implant prepares 3x less joint surface than the smallest posterior implant, i.e., 3 posterolateral implants will prepare an equivalent joint surface to 1 posterior implant. 2️⃣ Implant Placement: The largest posterior implant requires about 3x less bulk bone volume than a single posterolateral implant to be removed for placement. 3️⃣ Fixation: Both implants significantly stabilize lateral bending motions by 27%-28%. However, the smallest posterior implant stabilizes nutation-counternutation by 42%, which was 3x larger compared to the single posterolateral implant (14%) and two posterolateral implants (19%). 4️⃣ Long-term durability: Upon fatigue loading, both implants maintained stability in lateral bending at 12%-16%. However, at 0%, the single posterolateral implant lost all fixation in nutation-counternutation, while the small posterior interposition implant maintained fixation at 28%. 5️⃣ Bone-implant interface integrity: The smallest posterior implant showed no signs of interface failure, and its fixation was significantly correlated with bone quality, with axial rotation being the most sensitive and nutation-counternutation being the least sensitive. In contrast, 50% of the single posterolateral implants migrated, and 20% collapsed surrounding bone, with no correlation between fixation and bone quality. The fusion techniques were studied in a worst-case model. Future efforts include industry collaborations on the most generalizable models. This work was funded by Aurora Spine™ | Purpose Driven.
