Any software put out there into production is supposed to be "reliable". The reliability can be in any level or module of the software. One way to ensure reliability is to test thoroughly with all the known and possibly calculated edge cases. Also need to ensure there is no "Single Point of Failure". One needs to incorporate techniques like maintaining master and slave servers so if the master is down immediately the slave comes into the action. This is just one example. It is a vast field in itself :)

To improve the reliability of a software product, I believe that multiple testing procedures and approaches should be used. The testing process should be carried out from many perspectives, such as developer, tester, and end users. Another step is to test the product in a variety of scenarios to ensure that it works properly in all situations. In today's AI-powered world, I believe merging testing methods with AI from traditional testing methods would provide additional advantages in software testing and help detect defects and errors that may be difficult to find in traditional testing methods.

In my opinion, various AI tools can be utilized to conduct the software validation and verification.By AI coming into play , more and more positions will be created to validate the codes written by the open AI tools

With a proper architecture and properly defined semantics and conventions (variable and function naming, classes naming, code formatting etc), it becomes way more easier to ensure software reliability across a team, as all team members are on the same page in the context of "Software Engineering". Following TDD is the best thing that can happen to a software development team, as it becomes much easier to locate bugs and validate others code.

If you can detect the errors that would cause a fault, there's a good chance that you can correct the root cause and not have to handle it in the future. Validate inputs and pre-conditions, and do paranoid checks on your outputs. Log any violations. Read those logs! Don't rely entirely on unit tests. Make your code validate itself in normal use. Any problem that you catch and fix at development time is a problem you don't have to handle when the application is deployed with the customer. After that, the only problems you have to handle are those that come from external sources.

You can also validate the software properly as well as test the software thoroughly. You can get your code reviewed. You can remove any unused blocks of code. You can check the performance of your software using various tools which are available out there. These are some ways to ensure software quality assurance.

I'd like to add emphasis to code readability, under this umbrella of software quality assurance. Although it is not systematically measurable, easily readable code enables swift and accurate modifications (aka maintenance, and thus coupled with point #5- "SW maintenance [...]"). I've seen software bugs be introduced in maintenance (fixing other bugs, adding features, optimizing...). Often the new author doesn't fully understand the original design intent or the impacts of the changes they introduce. Many of the points mentioned in the SW quality assurance already lead to good readability. My point here is to explicitly call out "readability" as one of the goals behind the individual aspects like best practices, methodologies, and guidelines

Risk based testing, canary testing in production where a small segment of users or requests are exposed to the newly released code, can all help get more for less while reducing risk at the same time.

Unfortunately, this step is extremely rarely considered, designed and planned. As an example, think about flights, would you like to use end-of-life airplane?

Its useful to keep in mind that the term software originated as an easier alternative to changes in physical wiring. Frequent changes inherently decrease reliability, as you lose track record of real-life testing. If you want reliable programs they should be designed starting with a mathematical model that is then implemented in a computer system. The mathematical model allows to design a comprehensive testing suite which insures a certain level of performance. Changes after deployment are rare and subject to extensive scrutiny (the first being do we really need this change ?). None of this is cheap, which is why today we have software with an unending stream of patches.
