The first thing you need before youc an troubleshoot is to know the what and the how, in other words: requirements. What and how does your product need to perform? Once you have a clear (and agreed) view on that, and you begin testing properly, there's quite a few tools you may use, such as root-cause, 5-Whys or 8D. Consider each situation carefully, there is not a tool that always works for everything, so understand your problem and apply the correct tool for each situation!

Define the problem properly. The question I always use is...What was my expectation and what am I seeing. This then drives a clear problem statement.

A invenção ou inovação pressupõe-se uma solução para um processo, uma carência ou deficiência. Por si só ela deve ser vista como uma solução para um problema. Com base nisso há que se manter o foco no que se busca resolver ou melhorar. Um problema com a rota inicialmente proposta não pode tirar o foco do problema que se busca resolver com a invenção/ inovação. Em outras palavras, a invenção ou inovação tem como objeto o problema a ser resolvido e não um apego à hipótese de solução em si. Assim, podemos “pivotar” para buscar uma solução bastante distinta com o mesmo foco, ou inovar dentro do processo de inovação/invenção, um zoom, agora com foco na solução do problema do próprio processo inicialmente estabelecido.

The hardest part is if there are different explanations for an occurrence. Problems arise when a theoretical mechanism/action is based on an assumed cause/mechanism. With some problems you have to keep asking if your assumed cause/mechanism is the only one, i.e. fully recognize that it is a hypothesis, and keep asking what other causes or mechanisms could there be. i.e. if you have only one logical reason/hypothesis and it isn't responding to changes, keep looking for others. This can happen in more complex systems. Having staff with a wide variety of experience always helps.
