Registration Form

Please use the registration form if you wish to register by fax to one of our training workshops or program.

To register by fax:

  1. Download the REGISTRATION FORM
  2. Fill out the form
  3. Return it by fax at 613-562-5947.

Within three business days you should receive the official confirmation of your registration by e-mail. If it’s not the case, please contact us: 613-562-5802 or continue@uOttawa.ca.
