We shape, strengthen and sustain a health system that works for people, to achieve better healthcare particularly for those at risk of poor health outcomes. We are responsible for planning, guiding and directing investment towards important primary healthcare services. We identify priorities, link services, and fund care that is locally accessible, by working together with GPs, service providers, government and its agencies, and communities. We do this as the operator of the state’s three Primary Health Networks, which form part of the Australian Government’s national PHN program. We fund a range of primary healthcare services that helps more people access the care they need closer to home. We are delivering better health, together.

The Initial Assessment and Referral Decision Support Tool helps to recommend the most appropriate level of care for a person seeking mental health support. It is designed to be used by GPs and other clinicians alongside a comprehensive, holistic mental health assessment to gather information and guide referrals. It can also assist service providers, intake teams and PHNs responsible for undertaking initial assessments which may involve making recommendations on the level of care required.
