Dr Aaron Chambers is a GP and the cofounder of Growlife Medical in QLD, our 2024 winner of General Practice of the Year. In his extensive experience as a GP and practice owner he’s become a strong advocate for Multidisciplinary Care Teams, and explains that greater investment in this area will improve care and reduce costs, particularly for those living with chronic illnesses. “Delegating parts of a patient’s care makes sense where the patient needs extra support. Behaviour change and maintaining motivation also requires ongoing support. Creating a team around the patient helps to achieve this. It also means different specialist input: things like a dietitian adapting dietary advice or a diabetes educator helping a patient better understand how to adapt insulin and exercise. Having a pharmacist in our practice has been fantastic! It allows us to catch medication safety issues, avoid too many medications being prescribed, and have an internal expert who can provide advice to both patients and GPs.” As Australia faces an epidemic of chronic illnesses, we are asking the government for greater funding for Multidisciplinary Care Teams in order to improve health outcomes and reduce hospital admissions.
